Americans considering Phuket usually ask about the flight, the visa, Medicare, the IRS and whether they can own anything, in roughly that order. This article answers them plainly, with a date on every rule. It is a second base we are describing, not a move and not a promise.
The flight is the hard part
There is no non-stop from the United States. A single connection takes 20 to 29 hours door to door. The fastest routing we found from Los Angeles runs via Hong Kong in about 20 h 25 min, while Emirates options show 25 h 30 min and 29 h 20 min. We could not verify US-leg timings to Seoul, Taipei or Tokyo from the main US airports, and we found no non-stop between Tokyo and Phuket, so check live timetables.
The good news is the Asian side. Phuket has non-stops to Seoul Incheon (Korean Air and Asiana, two a day), Hong Kong (Cathay and HK Express, three a day), Singapore (nine a day) and Taipei (two a week). Korean Air's Phuket-Seoul flight takes 6 h 10 min, and 5 h 55 min from 25 October 2026. Plan one long travel day and a night to recover.
Phuket is 11 to 15 hours ahead of the US mainland, depending on the zone and daylight saving, so video calls with us fall in your evening and our morning.
The visa-free stay is 30 days
From 15 September 2026 US citizens get 30 days visa-free, for tourism only, with an extension of up to 30 more days at an officer's discretion. Land-border entries are reported to be limited to two a year. Some third-party sites still show the older, longer rule, so rely on official sources. For a winter you need a visa. The usual options:
- Destination Thailand Visa, ages 20 and over: THB 500,000 in savings, a THB 10,000 fee, five years, 180 days per entry plus one 180-day extension for THB 1,900. It does not allow work for Thai clients, and 180 days or more makes you a Thai tax resident.
- Retirement O-A: age 50 and over, THB 800,000 in a Thai bank or an income test, and insurance of THB 400,000 inpatient and THB 40,000 outpatient (agent guides; confirm with the embassy). O-X for five plus five years asks for a THB 3 million deposit or THB 1.2 million income, per one guide aimed at Americans.
- Long-Term Resident, Wealthy Pensioner: age 50 and over, passive earnings of $80,000 a year, or $40,000 to $80,000 with $250,000 in Thai assets, health cover of $50,000, a THB 50,000 fee for ten years.
- Thailand Privilege membership: no income test, Gold at THB 900,000 for five years, though sources differ on tiers.
Medicare stops at the border
Medicare.gov says Medicare usually does not cover health care while you are travelling outside the US, and Part D does not cover drugs bought abroad. So the question is insurance, not Medicare. One promotional page quotes cover for people over 60 from about THB 35,000 a year; treat that as a starting point for quotes, not a price.
Hospitals
Bangkok Hospital Phuket is a full-service, JCI-accredited hospital with more than 300 beds and English-speaking specialists (according to an expat guide; we did not check the JCI directory ourselves). Complex cases go to Bangkok, about 1.5 hours by air, where Bumrungrad became the first JCI-accredited hospital in Asia in 2002. We found no independent study comparing prices with US hospitals, so ask for written quotes and compare yourself. For safety, read the State Department's current advisory for Thailand at travel.state.gov rather than relying on our paraphrase.
What an American can own
Foreigners, Americans included, cannot own land. A condominium unit can be owned freehold in your own name within the building's 49% foreign quota. Land and villas are held on a registered lease of up to 30 years, renewable by contract. Nominee companies are under enforcement in 2026, so we advise against them. Ask for written confirmation that the quota has room.
What the IRS wants to know
This is general, and your preparer decides. FBAR applies when your foreign accounts together exceed $10,000 at any time in the year, filed with FinCEN. Form 8938 for taxpayers living abroad applies above $200,000 at year-end or $300,000 at any time for a single filer, and $400,000 and $600,000 for joint filers. A foreign condominium owned directly is not reportable on either form; held through a foreign entity, the entity is. Rent from it is reported on the US and Thai returns. The foreign earned income exclusion is $132,900 for 2026.
Thai residents, meaning 180 days or more a year, owe Thai tax on foreign income brought into Thailand since 1 January 2024. A relief for income remitted within two years was proposed in 2025 and, as of March 2026 guides, not yet enacted. Long-Term Resident holders are exempt on foreign-sourced income. Social Security is not taxed by Thailand under the treaty, and there is no totalization agreement.
A month in dollars
One agency estimate from April 2026 puts a comfortable month at $1,800 to $3,000, with one-bedroom rents of $700 to $1,500, and December to March 30% to 50% dearer. The dollar-baht rate ranged from 30.9 to 33.8 in 2026, so budget with a margin. UWC Thailand teaches the IB from nursery to Grade 12, with about 500 students from more than 60 nationalities.
How we meet
Vika Thai Company Limited is an agency in Phuket, working since 2013, and the developer pays our fee. Managers speak English, Russian and Turkish. A call is 20 minutes by video at an hour that suits you, then a shortlist with prices, and a viewing day when you can fly. The US page shows mid-range and premium residences from $150,000.
